Florida City Dental respects your privacy and is committed to protecting your health information.

 

 

1) Website Forms and Messages

 

When you submit a form on our website (such as an appointment request or contact form), the information you provide is used to respond to your request and help schedule care.

 

Please do not submit sensitive medical information through general website forms unless a form specifically requests it and indicates it is intended for that purpose.

 

Not for emergencies: If you believe you have a medical emergency, call 911.

 

 

2) Protected Health Information (PHI)

 

If you become a patient, information we maintain as part of your dental care may be considered Protected Health Information (PHI) and is handled in accordance with HIPAA and other applicable laws.

 

In a clinical setting, patients typically receive a separate Notice of Privacy Practices that explains:

 

• how your PHI may be used and disclosed,

• your rights regarding your PHI (access, amendments, restrictions, etc.),

• and how to file a privacy complaint.
